What this form is for

• Claim maternity allowance under the loss-of-earnings compensation scheme (EO/APG), which replaces part of your income after giving birth.
• The other parent has their own form (318.747) for other-parent leave.

Documents to have ready

• Your AHV/AVS number and bank details
• The child's date of birth (birth certificate or confirmation)
• Details of your work and salary (provided by your employer, if you have one)

Where to send it

Employees usually apply through their employer, who passes it on to its compensation office. Self-employed or unemployed: send it directly to your compensation office.
