What this form is for
- Get a criminal record extract for private individuals, which shows whether any convictions are recorded in your name.
- A special extract exists for activities involving contact with minors or vulnerable people: it requires a signed confirmation from the employer or organisation.

Where to send it
Only online on the Federal Office of Justice's official site (casier.admin.ch) or in person at a Swiss Post counter. If you order online, you then print and sign the generated form and post it with a copy of your ID.
